Change Today for a Healthier Future Diabetes Prevention Program
Thursdays: April 20, 2017 – March 20, 2018
11:00 a.m. to 12:00 p.m.
Work with others in a small group setting, for a one-year commitment, to learn how to adopt habits that will improve your overall health and well-being and reduce your risk for diabetes.
Programs like the YMCA’s Diabetes Prevention Program have been shown to prevent or delay type 2 diabetes in individuals by 58%. The YMCA’s Diabetes Prevention Program is part of the Center for Disease Control and Prevention-led National Diabetes Prevention Program and is nationally supported by the Diabetes Prevention and Control Alliance.
The class meets weekly for sixteen weeks, bi-weekly for 3 months, and monthly for the last six months of the program.
• Age 60 or older, at or below 185% of the Federal poverty line: no cost if enrolled through Heritage Place by March 30, 2017.
• Self-pay option.
